INSTRUCTIONS FOR PREPARATION AND USE (PARAAN NG PAGHAHANDA AT PAGGAMIT)

Your baby’s health depends on carefully following the instructions below. Proper hygiene, preparation, dilution, use and storage are important when preparing infant formula. Powdered infant formulas are not sterile and should not be fed to premature infants or infants who might have immune problems unless directed and supervised by your baby’s doctor. Ask your baby’s doctor which formula is appropriate for your baby. Ask your baby’s doctor about the need to use cooled, boiled water for mixing and the need to boil clean utensils, bottles and nipples in water before use.

1. Wash hands thoroughly with soap and water before preparing formula. Hugasan nang mabuti ang kamay gamit ang sabon at tubig bago ihanda ang gatas.

2. Boil clean bottles, nipples, caps, and utensils in water (1 minute at a rolling boil). Pakuluan ang malinis na bote, tsupon, takip at iba pang kagamitan sa kumukulong tubig ng 1 minuto.

3. Boil fresh water at a rolling boil for 1 minute. Cool to room temperature prior to mixing. Pakuluan ang malinis na tubig ng 1 minuto. Palamigin ang tubig bago ihalo.

4. Pour desired amount of the cooled water into the bottle. Add powder. Ibuhos ang tamang sukat ng pinalamig na tubig sa bote. Ihalo ang gatas.

5. Cap bottle and SHAKE WELL.Takpan ang bote at ALUGIN NANG MABUTI.

Feeding TableUse the chart below for the correct amounts of cooled, boiled water and powder. Use scoop in can to measure powder. Store DRY scoop in this can.

One level scoop (approximately 4.3g) of Nutramigen LGG Infant Formula powder to 30mL of cooled, boiled water.Normal Dilution: 13.6g of Nutramigen LGG Infant Formula diluted with 89.7mL of cooled, boiled water to make a 100mL reconstituted product.

POWDER STORAGE

Store can at room temperature. After opening, keep can tightly covered. Store in a cool, dry place to esnure freshness. Use contents within 1 month after opening. Use this product before the expiry date indicated at the bottom of the can. Do not freeze powder and avoid prolonged storage at excessive temperatures.

WARNING

Do not use a microwave oven to warm formula. Serious burns may result. Failure to follow these instructions could result in severe harm to your baby. Once prepared, infant formula can spoil quickly. Either feed immediately or cover and store in refrigerator at 2 - 4°C for no longer than 24 hours. Do not use prepared formula if it is unrefrigerated for more than a total of 2 hours. Do not freeze prepared formula if it is unrefrigerated for more than a total of 2 hours. Do not freeze prepared formula. After feeding begins, use formula within one hour or discard.

1. Please do not consume if there is any packaging defect or if pouch is not sealed.

2. If reconstituted beverage is not consumed immediately, keep in a refrigerator and usewithin 24 hours after mixing.

3. Once feeding begins, use formula within 1 hour or discard.
